This notice explains how information is handled through the VEDI website and its patient referral form. The form is intended for veterinary hospitals submitting information for diagnostic imaging and case coordination.
Information you provide
A referral may include hospital and veterinarian contact details; an owner’s name and phone number; patient identification, birthdate, species, breed, sex, weight, and reproductive history; the requested service; clinical history, medications, and diagnostic questions; and uploaded labwork or patient records.
Submit only information relevant to the referral that you are authorized to share. Remove unrelated personal information from attachments. Do not include payment-card details, Social Security numbers, or human medical records.
How referrals are used and delivered
Information is sent to VEDI to review the case, communicate with the referring hospital, and coordinate diagnostic services. The referral email includes submitted information and attachments, and uses the referring hospital’s email address as the reply address.
Cloudflare hosts and processes the website request. Resend processes the referral email for delivery to VEDI’s designated inbox, currently laurie@vediultrasound.com. After the referral is accepted for sending to VEDI, a submission confirmation and copy of its details and attachments are also sent to the hospital email address entered on the form. Check that address carefully before submitting. The receiving email providers also process and store the messages. These providers handle information under their own terms and privacy practices.
The form does not publish referrals or use their contents for advertising. A successful submission confirms acceptance by the sending service; it does not confirm that VEDI has read the referral or scheduled an appointment.
Storage and downloaded copies
The website sends referrals by email and does not maintain a separate patient-referral database. Submitted information and attachments can remain in email systems, provider records, and backups. This form does not automatically delete those copies after submission.
If you download a referral copy, it contains the submitted details, a submission reference and time, and attachment filenames and dates. The attachments themselves are not embedded. Keep the downloaded file securely in your hospital’s records and avoid saving it on shared or public devices.
Website visits and outside services
Website and delivery providers may process technical information such as an IP address, browser information, request times, and delivery or error records to operate and protect their services.
Externally hosted photos and video load from outside services, including Wikimedia, Pixabay, and Pexels. The service-area map loads its street background from Esri / ArcGIS Online. Its service boundaries and fallback coastline are bundled with this website. Loading that content can share technical information about the visit with those services. Links to other websites are subject to those websites’ privacy practices. Using the Text VEDI button opens your device’s messaging application; messages are handled through your messaging service and VEDI’s receiving service.

VEDI can discuss what can be changed or removed, including any records that need to be retained. Please identify the referral without sending unnecessary sensitive information.
